REHP PEBTF is providing coverage for at-home COVID-19 tests for Medicare eligible members currently enrolled in the SilverScript Part D prescription drug plan. You can have your local retail store process the claim at the pharmacy counter by showing your current SilverScript ID card or you can submit a manual claim for reimbursement. For instructions to complete an electronic manual reimbursement, please Click Here. To print and mail in the paper manual reimbursement, please open the Claim Form and complete. Any test kits purchased on or after 1/15/2022 are eligible for reimbursement

The paperless online EOB is safe and secure and provides the same information in the same format as the paper EOB that you receive in the mail. You will be notified by email when you have a new EOB to view. To get started, visit Caremark.com to register. You can opt in or opt out of paperless online EOBs at any time.
